1. I confirm my pet is current on vaccinations (including rabies, if applicable).
2. I have disclosed any health conditions, skin issues, or behavioral concerns.
3. I understand grooming may aggravate previously hidden conditions (skin irritation, hotspots, etc.).

• I authorize Dog On Cute Pet Grooming to bathe, groom, clip, and/or trim my pet as discussed.
• I understand grooming tools (clippers, scissors, dryers, etc.) are used and accidents may occur. Groomers will take all precautions to ensure safety.
• If my pet is severely matted, I understand shaving may be necessary and that skin irritation or nicks may result.

• In the event of illness or injury, I authorize Dog On Cute Pet Grooming to seek immediate veterinary care at my expense.
• I release Dog On Cute Pet Grooming from liability for any pre-existing conditions or unforeseen health issues during grooming.

• I agree to pick up my pet promptly at the agreed time.
• I understand that repeated late pick-ups or no-shows may result in additional fees or refusal of service.

• I agree to hold harmless Dog On Cute Pet Grooming, its staff, and agents from any claims, damages, or expenses arising from services provided.
• I acknowledge grooming can be stressful for pets and results cannot be guaranteed (coat length, behavior, style, etc.).
